Qualcomm Life, Inc. and AMC Health recently announced a partnership to help accelerate improvements in the way population health management is carried out through enhancing connectivity and coordination of at-risk patients with diseases like COPD, asthma, heart failure, diabetes and hypertension using a cost-effective approach. The companies will utilize Qualcomm Life’s 2net™ Connectivity Platform together with AMC Health’s CareConsole™ care coordination platform.
The 2net Hub, 2net Platform and 2net Mobile are innovative solutions for enabling seamless recording, transmission and collection of biometric data for a wide range of FDA-listed medical devices, almost in real time. 2net allows increased interoperability plus flexibility to record device data for all types of patients while they are at home or on the go. It also securely transmits data to AMC Health’s CareConsole platform. In turn, the CareConsole platform includes an advanced analytics system to collect, verify and analyze data that also presents clinically actionable and daily patient information in an extremely user-friendly format so clinicians can easily use it. Additionally, the CareConsole also warns clinicians about emerging health issues, and enables real-time education, focus and attention to assure care to patients.
